On the Same Page is a full-color 2-sided 11 X 17-inch tri-fold form that accompanies the Design to Learn package of materials. It is used for integrating assessment information from home and school and for developing and monitoring an integrated learning plan for an individual student. This form provides space to enter the results of communication and concept development skills derived from administering the Communication Matrix and the HIPSS and SIPSS at home and at school as well as child preference information.
